How do I file a BOC-3 form?
Form BOC-3 (Designation of Process Agents) must be filed by the process agent or blanket process agent you choose. Only one completed form can be on file with the FMCSA.
You must designate a process agent in every state where you operate or travel through. Each agent must have a physical address in that state so legal documents can be delivered during normal business hours. P.O. Boxes are not accepted.
